Wheel Clean DFW Photo & Video Consent

By checking the box below, you consent to the following:

1. Wheel Clean DFW may take photos and/or videos of your wheelchair or mobility aid before and after service.

2. Wheel Clean DFW may take photos and/or videos that include you (the client) and those in relation to the services performed. For all minors under 18, faces and identifying features will be thoroughly covered or blurred.

3. Your first name only may be used in captions or descriptions (example: “David’s power chair”).

4. Consented photos/videos and those provided by Client may be published on our website (wheelcleandfw.com) and social media (Instagram @wheelcleandfw and Facebook).

Additional terms:

• Consent is voluntary and may be declined for any reason.

• Materials will be used for promotional purposes only. No payment or compensation is provided.

• You may withdraw consent at any time by contacting Wheel Clean DFW (wheelcleandfw@gmail.com). We will make reasonable efforts to remove previously published materials upon request.

• All minors will be de-identified if any client photos are used.

• By checking the box you confirm you are the client or the parent/legal guardian authorized to give this consent.
